Who will make decisions if I cannot?

If there are problems after your heart surgery, you may not be able to make decisions about your own care. In the unlikely event that:

  • You are unconscious or in a coma

  • You are confused or delirious

  • You may have had a stroke

  • You may need prolonged assistance with breathing via a ventilator

How would you want decisions to be made for you about your ongoing health care, living arrangements and other personal matters? And, who would you want to make those decisions for you? Your surgeon will speak with you and your family about any important decisions that have to be made, but you can prepare ahead with an Advance Care Directive.

An Advance Care Directive gives those who care about you a clear understanding of your needs and wishes should you lose your ability to communicate them for yourself.

An Advance Care Directive replaces existing Enduring Power of Guardianship, Medical Power of Attorney and Anticipatory Direction.
What will it allow me to do?

An Advance Care Directive allows you to:

  • Write down your wishes, preferences, and instructions for your future health care, end of life, living arrangements and personal matters; and/or

  • Appoint one or more Substitute Decision-Makers to make these decisions on your behalf, if you are unable to make them for yourself.

Why should I have one?

It will give you peace of mind that those caring for you will know what you want when you are unable to make your own decisions or communicate your wishes and values.

What does the kit contain?

The kit contains the Advance Care Directive Form and a step-by-step guide that gives you everything you need to complete your Advance Care Directive Form, including example statements, information for Substitute Decision Makers, witnesses and interpreters, fact sheets and other resources and links.
